Amber heat health alert 8-12 July

The UK Health Security Agency (UKHSA) has issued an Amber Heat Health Alert for all regions in England, from 9am on Wednesday 8 July until 9pm on Sunday 12 July. Rising temperatures may increase health risks, particularly for vulnerable groups. What you can expect from the Care Experienced Service

From the age of 16, you should be allocated a Personal Advisor (PA) to work with you to ensure you receive the support you need.

Your PA will work alongside your children’s social worker until you reach 18. At 18, your PA will continue to support you up to the age of 25 if needed.

We will:

  • Complete an assessment of your needs to ensure the correct services are available to you.
  • Develop and put in place a Pathway Plan detailing the support you need to achieve your goals.
  • Ensure that you experience an inclusive service; enabling any disabilities and/or additional needs to be considered as part of pathway planning with you and partner agencies
  • Provide financial advice and guidance.
  • Provide accommodation until you are 18 and housing advice for after that.
  • Provide education, training, and employment advice and support.
  • Provide a health passport and support with health and wellbeing issues.
  • Provide advice on social activities.
  • Keep in touch regularly (minimum of every 8 weeks).
